We look forward to seeing you. Please bring your valid ID, insurance information, and credit card to your appointment. Please note: we require all patients to keep a valid credit card on file for faster checkout and efficient billing. Thank you, we appreciate your cooperation.

Payments

Cosmetic & Aesthetic services are typically not covered by insurance, therefore full payment for Cosmetic & Aesthetic services is due at the time of the visit. 

For Medical services any copays or deductibles are due at the time of the visit. After your insurer has paid their portion, they will indicate any remaining payment or refund that is due. For a remaining balance less than $50, we will bill your credit card on file. For remaining balances over $50, we will send you a statement. If payment is not received within 15 days the remaining balance will be billed to your credit card on file. 

Please examine your statement carefully. If you feel you were billed in error, immediately alert the office to resolve the issue or your account will be considered delinquent. Delinquent accounts may be subject to interest and collection fees.

For your convenience, accounts can be paid in the office, by telephone, or online using your Mastercard, Visa, Discover, American Express and Health Saving credit card.

Appointment Cancellations

We understand that things come up and we are happy to cancel or reschedule your appointment with at least 24 hours notice. This allows us sufficient time to reallocate your appointment time to other patients on our waitlist.

  • A SAME-DAY CANCELLATION will be assessed a late cancellation fee of $50

  • A MISSED APPOINTMENT will be assessed a no-show fee of $50

New Patients

We are a paper-free office so all new patient paperwork will completed electronically at the time of your first visit. If you need help, just ask! Please arrive 10 minutes ahead of your visit to complete check-in so your appointment can begin on time.

Please bring with you:

  • Valid Photo ID

  • Insurance Cards (primary & secondary)

  • Valid Credit Card (to be kept on file - see Financial Policy for further information)

  • HSA Card

  • List of Current Medications

Patient Portal

After your first appointment you will be emailed a link to set up your online portal. Open the emailed link to set up your username and password for the Patient Portal. This portal will allow you to update your health history, check for biopsy results, and confidentially communicate with our staff in the future. If you are having trouble with your portal, please call our office. 

Illness

At DeRosa Dermatology our highest priority is the health and safety of our patients, staff, and community. Following state guidelines, mask wearing is optional. We urge any patient who is feeling ill with fever, cough, or other respiratory symptoms to please reschedule their appointment; please provide 24 hours notice so we can appropriately manage our staff and schedule.

When visiting our office, please do not arrive more than 10 minutes early to your appointment and please refrain from bringing additional individuals to your appointment, unless the patient is a minor, or the patient has a physical limitation that requires aid.
